DEADLINE FOR FORMS TO DR. MATHIAS: Early July 2011- Medical forms - Each Scout and leader staying in camp more than 24 hours must have a completed medical form on file at the Camp Health Lodge. Effective 2010, BSA requires a physical evaluation be completed annually for adults and Scouts attending resident camps. A health form signed by a licensed health care provider (MD, DO, NP, or PA) and dated within one year of the week attending camp must be on file at the camp’s medical facility. Make sure your son's doctor signs and dates the form (should be the date of the last physical) or it's not acceptable. Forms signed by the nurse or receptionist are not acceptable. Alternate forms (e.g., school health form) are not acceptable. Each medication must be signed by the parent and doctor (where it says "Distribution approved by:"). Otherwise Nurse Dave won't take it. Honest.
